EVSC 691 Directed Studies Course Form

• You must also include a course syllabus that has been agreed to between you and your supervisor.
• If submitting past week 5 of the term, please include an Add Course form (see Graduate Studies Forms, below)
• Directed Studies application must be sent to the Graduate Program Assistant at least three (3) weeks prior to a term start.
